Should I address my employer as "Dear Sir or Madam" or should I write " Dear 'Mr. Smith / Mrs. Jane' ?

"Mr. Smith" is the appropriate choice because you definitely know the name of your boss.
You use "Sir/Madam" only when you don't know the name of the person who you are writing to.

I think, Dear Sir/Madam is the proper one. General salutation is enough to address our employer.
